- Photo ID
- Referral authorization from your primary care physician (if required)
We accept cash, checks with appropriate ID, and most major credit cards.
To reserve a laser or cosmetic dermatology appointment, a credit card number is required.
All minors are required to have parental/guardian permission for each appointment.
Cancellation Policy:
Since we reserve our provider’s time exclusively for you, we require a minimum of 24 hours notice to cancel an appointment. Cancellation fees are based on the procedure and the time commitment for the appointment.
